I am twenty five years & five months, of age. My birth place was Worcester Massachusetts. I graduated at Brown University in the class of 1859. I attended lectures last winter at the University of Pensilvania & have studied midicine two years. Dr. Henry W. Rivers of Providence R.I. was my instructor with whom it was my privalege to witness, in the lower ranks of society, surgery & practice. I also had frequent oportunities of attending the asylum in Providence with Dr. Ely, the attending physician. When persuing the study of materia medica I examined the drugs at an apothecary shop, to some extent. I am very little informed on natural history. I once studied latin, greek & german but am little familiar with them now.
